Membership Information
Instructions for applying for membership:
1. Go to MEMBERSHIP APPLICATION page.
2. Print form by clicking 'File, Print.' It will print two pages to your default printer.
3. Fill out form as completely as possible. Don't forget to sign it. Email us with any questions.
4. Enclose a check payable to Pontiac Lake Horseman's Association (PLHA)
5. Mail signed form and check to:
